Plan of Care

A Case Manager will visit your home to do an assessment and obtain critical information to identify the particular challenges and needs, as well as the health and functional status of the elderly client.
The Case Manager will then develop a plan of care and diligently match a caregiver that will have the capacities to provide care to the client.

We carefully assess and attentively listen to the values, preferences and concerns of the client and the family. The care provided is tailored upon this assessment.
Our comprehensive evaluation is the cornerstone of our care plan and communication is the key to a good and successful relationship.

A Case Manager is assigned to every client who will coordinate between the client and caregiver to make sure that the care provided is consistent and of quality.
The Case Manager will make periodic calls and visits. She goes through the progress notes being maintained by the caregiver.
